Workers Compensation Program—Medical Provider Network

San Diego County JPA provides workers compensation coverage for the District and its employees.  The JPA is making changes to the Medical Provider Network for all dates of injury occurring on or after 11/1/08.

Employee Pre-designation Form

The acknowledgement of the MPN & pre-designation forms must be sent back to the District’s Risk Management Department.  The pre-designation form must be on file prior to any injury.  Your designated physician must also agree to provide the services required which the District will verify.
